Regardless of format type, each line should contain the following:

	Input Format
	domain-name host-name
	domain-name new-host-name

where:
	domain-name	is the fully-qualified domain name
	new-host-name	is the fully-qualified host name that the domain name points to
	new-host-name	is the new fully-qualified host name that the domain name points to
