Last Name		 Needed to register for the Net-ID. Goes in online directories (e.g., the Exchange Global Address List [GAL]).
Birthday Needed to register for the Net-ID. If you select "Unknown", we'll just make one up.
Affiliate ISU-ID If the individual is eligible for an ISU-ID assigned by the ISU Card Office, we recommended obtaining one and using it here. This will greatly ease a future transistion to a Normal (unsponsored) Net-ID should the person become eligible for one.

If you select "Not Eligible for ISU-ID", we'll just make one up.

Affiliate Net-IDs with "made up" ISU-IDs do not appear in the Online Directory nor are they supplied to any business systems.

Email Address This is used only to send to the individual the instructions on how to register for an ISU Net-ID.
Directory Title Goes in online directories.
Department to Place In Goes in online directories.

Used to place the Affiliate Net-ID in the appropriate Active Directory "Organizational Unit" [OU].
