Last Name		 Needed to register for the Net-ID. Goes in online directories (e.g., the Exchange Global Address List [GAL]).
Birthday Needed to register for the Net-ID. If you select "Unknown", we'll just make one up.
ISU-ID Needed to register for the Net-ID. The entire 17 digit ISU-ID is needed to verify that it is a valid ISU-ID.
Non-ISU Email This is used only to send to the individual the instructions on how to register for an ISU Net-ID.
Payclass Used to determine proper list/group membership.
Job Title Goes in online directories.
Department Used to determine proper list/group membership. Goes in online directories.

Used to place the Affiliate Net-ID in the appropriate Active Directory "Organizational Unit" [OU].
